Why do I need to attend an in-person annual Health Plan check-in to qualify for the Healthpoints benefit?
Regular in-person Health Plan check-ins are important as they allow your Healthier SG doctor to assess your health in-person and provide recommended screenings and vaccinations during the same visit where appropriate. Annual Health Plan check-ins can be conducted in person or via tele-consult, and how often you should return to your Healthier SG clinic for an in-person annual Health Plan check-in depends on whether you have any chronic conditions.
a. If you are generally healthy with no known chronic conditions, you are recommended to check-in with your Healthier SG doctor once a year. The check-in may be done in-person or over the phone (your Healthier SG clinic will advise on the mode of check-in). An in-person check-in must be done once every three years.
b. If you have one or more chronic conditions, your doctor may recommend two or more check-ins within the year to better review and manage your condition(s). An in-person check-in must be done at least once a year.
