In-person Health Plan check-ins completed before 1 July 2026 do not qualify for the one-off 1,500 Healthpoints benefit (worth $10), even if you have viewed your Health Plan after the check-in. To qualify for the one-off 1,500 Healthpoints benefit (worth $10), both the in-person annual Health Plan check-in and viewing of Health Plan on HealthHub after the in-person check-in must be completed within the qualifying period of 1 July 2026 to 31 December 2029.

Annual Health Plan check-ins can be conducted in person or via tele-consult, and how often you should return to your Healthier SG clinic for an in-person annual Health Plan check-in depends on whether you have any chronic conditions:
If you are generally healthy with no known chronic conditions, you are recommended to check in with your Healthier SG doctor once a year. The check-in may be done in-person or over the phone (your Healthier SG clinic will advise on the mode of check-in). An in-person check-in must be done once every three years.
If you have one or more chronic conditions, your doctor may recommend two or more check-ins within the year to better review and manage your condition(s). An in-person check-in must be done at least once a year.
