Can I do my annual Health Plan check-in via tele-consultation after changing my Healthier SG clinic?
Your first annual Health Plan check-in after switching to a new Healthier SG clinic must be done in person. This allows your new doctor to assess your current health, address any concerns, and establish your ongoing care at the new clinic.
After this initial in-person visit, subsequent check-ins may be conducted via tele-consultation, depending on your doctor’s advice and your health needs.
For Healthy Individuals:
If you are generally healthy with no known chronic conditions, you are recommended to check in with your Healthier SG doctor once a year. These check-ins may be done in person or over the phone, as advised by your Healthier SG clinic. However, an in-person check-in must be done at least once every three years.
For Individuals with Chronic Conditions:
If you have one or more chronic conditions, your doctor may recommend two or more check-ins within the year to better review and manage your condition(s). An in-person check-in must be done at least once a year.
Your existing Health Plan remains valid and can be accessed in the HealthHub app. To continue your Healthier SG journey, schedule your fully-subsidised first annual check-in with your new clinic by the recommended date shown in the app.
Regular check-ins are essential for tracking your health progress and updating your Health Plan. If you have chronic conditions, these sessions will help manage your health, set targets, and implement interventions like weight management or smoking cessation.
