What “reasonable measures” should be taken to ensure continuity of care of affected patients in the event of cessation of service / transfer of care?

Some examples of “reasonable measures” could include contacting every affected patient within a reasonable period prior to cessation of service, putting up a notice on the website and/ or at the clinic, and/ or arranging transfer of care to another appropriate licensee (in such cases, the patient’s acknowledgement should be obtained).


Licensees may wish to seek legal advice pertaining to the handling of patient health records (e.g., if patient is uncontactable).
