A deed poll is a legal document executed by Advocates or Solicitors to effect a change in name. The deed poll should be executed by the registrant's parents if the registrant is below 21 years of age.
Deed Poll should be executed by both parents/ legal guardians. If the deed poll was executed by only one parent/legal guardian, either parent/ legal guardian must be present during the registration to make a declaration that both parents consented to the name change.
