Can I still qualify for the one-off Healthpoints benefit if I am unable to access HealthHub to view my updated Health Plan (e.g. I do not have a smartphone)?
You are encouraged to view your Health Plan on HealthHub as it includes resources to support you in working toward your health goals, such as reminders for recommended screenings and vaccinations, and suggested exercises.
If you have a caregiver who has been granted access to your Health Profile on HealthHub, your nominated caregiver can also help to view your Health Plan via HealthHub on your behalf after your in-person annual Health Plan check-in. Viewing by a nominated caregiver will also qualify you for the one-off 1,500 Healthpoints benefit (worth $10). The one-off 1,500 Healthpoints benefit (worth $10) will be credited to your Healthy 365 account, even if your nominated caregiver viewed the Health Plan via HealthHub on your behalf.

If you are unable to access HealthHub, you can request for a hardcopy of your updated Health Plan from your Healthier SG doctor during your in-person Health Plan check-in. Your Healthier SG doctor will inform MOH accordingly, so that you qualify for the Healthpoints benefit. You are encouraged to view your hardcopy Health Plan as it captures your health goals and recommended follow-up actions.
