With Caregiver or Travel Card:

Use a family member’s or friend’s smartphone to create and access your Healthy 365 profile. After completing the Health Plan consultation with your doctor, you can check and receive your enrolment benefits.
Alternatively, visit Healthier SG Enrolment Stations, IMDA digital hubs, Active Ageing Centres (AACs), or call the HPB Hotline at 1800 223 1313 for assistance in setting up your Healthy 365 account.
Ensure you set up your account within 12 months of your first Health Plan consultation to receive 3,000 Healthpoints (worth $20).
Healthpoints are valid for 6 months and can be redeemed for e-vouchers, such as FairPrice eVouchers.

Without Smartphone or Caregiver (Travel Card Option):

If you do not have smartphone access or caregiver support, your 3,000 Healthpoints enrolment benefit will be automatically redeemed for a S$20 SimplyGo eVoucher which will be credited into a Travel Card of your choice.
Visit Healthier SG staff at selected polyclinics or public health institutions to assist you with the conversion by creating a Healthy 365 Lite account.
Bring your NRIC and a valid Travel Card (e.g., TransitLink card) for account creation.
