As part of the application, we would require you to fill in the following details:
Personal Information
Family / Next-of-kin living with you information
And for you to upload the following documents to help expedite the application:
Latest bank statements for you and your spouse's accounts
Latest payslips/CPF/IRAS statements for you and family
Any other supporting financial documents
Upon submission, our staff will review your application and may contact you for further information. Please attend your appointment as scheduled regardless of whether you hear from us or not - unless specifically told not to.

To apply for financial assistance, you must:
be a subsidised patient;
have received or require treatment from a Public Health Institution; and
be having difficulties paying for your healthcare bills after Government subsidies and other means including MediShield Life, MediSave, and cash.
Your application will be assessed holistically, taking into consideration you and your family’s financial, health and social circumstances.
