You can get it from the hospital or doctor.

In most cases, this can be found on a document issued by the hospital or doctor. This could be a confirmation of death, confirmation of stillbirth or a similar document. 

Depending on the document, the document number will come in the following formats:

• 6-digit number, followed by a letter (000001A) when the digital death certificate is ready for download

• 5-digit number, followed by a letter (01234A) when a digital stillbirth certificate is issued

• the letter D, followed by a 5-digit number (D10000) when a temporary death document is issued

• the letters SB, followed by a 4-digit number (SB0123) when a temporary stillbirth document is issued

If the death or stillbirth was certified outside of a hospital, you may receive a printscreen via WhatsApp instead of a printed document. However, the doctor will still provide you with the 3 information required to download the digital certificate.

If you did not receive the document number, contact the doctor or hospital within 30 days from the date of death or stillbirth.
