Where can I get the death or stillbirth document number?
You can get it from the hospital or doctor.
In most cases, this can be found on a document issued by the hospital or doctor. This could be a confirmation of death, confirmation of stillbirth or a similar document.
Depending on the document, the document number will come in the following formats:
6-digit number, followed by a letter (000001A) when the digital death certificate is ready for download
5-digit number, followed by a letter (01234A) when a digital stillbirth certificate is issued
the letter D, followed by a 5-digit number (D10000) when a temporary death document is issued
the letters SB, followed by a 4-digit number (SB0123) when a temporary stillbirth document is issued
If the death or stillbirth was certified outside of a hospital, you may receive a printscreen via WhatsApp instead of a printed document. However, the doctor will still provide you with the 3 information required to download the digital certificate.
If you did not receive the document number, contact the doctor or hospital within 30 days from the date of death or stillbirth.
If it has been more than 30 days since the date of death or stillbirth, please contact the Registry of Births and Deaths, ICA.
