deceased's NRIC, FIN, or passport number (as per what the doctor used during death certification)
death document number
date of death
For stillbirth certificate
You'll need to provide:
mother's NRIC, FIN, or passport number (as per what the doctor used during death certification)
stillbirth document number
date of stillbirth
These information will be given to you by the hospital or doctor.
