There are no costs involved when drafting an ACP. However, you will have to make an appointment with an ACP facilitator so that they can answer questions you may have and submit your ACP form to National Electronic Healthcare Record (NEHR). There will be fees involved in that session. You may check with the various ACP facilitators on information about the cost.
