For ACP, you will need to appoint up to 2 Nominated Healthcare Spokesperson (NHS). Your NHS should be 21 years of age or older. They could be:

• your husband, wife or partner

• a family member or relative

• a friend

In the event that you're unable to communicate, the medical team will consult your nominated healthcare spokespersons. They may be asked to represent your care preferences and speak on your behalf.
