Once both donor and CI have digitally signed on the LPA form, no amendments can be made. This is a safeguard to prevent tampering and alteration of the digital LPA. While we are not able to make any amendments to the LPA instrument itself, we would still need to rectify the error in our internal records to ensure factual accuracy. As part of the rectification, we would need a statement from you explaining the error made as document indicating (i) the affected online LPAs, Donor’s name and ID, (ii) date LPA was certified and, (iii) error on wrong profession (and elaboration on the reason if any).
