You can request for a formal SAF Medical Report through the eHealth portal.

Step 1: Log in to eHealth

Log in to your eHealth account at MINDEF eHealth.

Step 2: Go to the Home tab

On the Home tab, click on "Request for Medical Report".

Step 3: Fill in the request form

Complete the electronic request form with your details.

Step 4: Submit your request

Submit the form. Your application will be reviewed and you will receive an outcome — either Approved or Rejected.

(Note: An SMS will also be sent to your registered mobile number once your application is approved)

Step 5: Make payment (if approved)

If your application is approved, the payment amount will be shown. Make payment within 10 working days of approval. Your application will be cancelled if payment is not made within this period.
