Before online submission of medical screening questionnaire

If your endorser does not have SingPass, you may opt for the medical questionnaire to be endorsed via a hardcopy printout by following the below steps:

Step 1: Select the option "My endorser will complete the endorsement on a printout of the Medical Screening Questionnaire" under the Endorser tab

Step 2: Print the questionnaire at the last step on the confirmation page and pass it to your endorser to sign under Section D Please submit the printed hardcopy of the medical screening questionnaire, with your endorser's signature, on the day of your pre-enlistment medical screening.

After online submission of medical screening questionnaire

If your endorser is unable to proceed with the endorsement after you have submitted the medical questionnaire online, you will then need to prepare a hardcopy questionnaire for your endorser.Please download and print the questionnaire via the Endorse Medical Screening Questionnaire dashboard card on your OneNS dashboard and have your endorser endorse on the hardcopy questionnaire. Please bring this hard copy questionnaire on your pre-enlistment medical screening appointment and submit it during your registration. Please inform your endorser to ignore the SMS reminder for online endorsement.
