Please follow the steps below to submit your medical screening questionnaire online: 

Step 1: Log in to OneNS using your SingPass

Step 2: Look for Complete Medical Screening Questionnaire dashboard card on your OneNS dashboard 

Step 3: Click on "Medical Screening Questionnaire" icon

Step 4: Follow the instructions online to complete the different sections of the questionnaire

Step 5: You will see a "Verification" page which is a summary of the questionnaire. After verifying your entries, click on "Submit"

Step 6: A confirmation will be shown upon submission. If you have indicated that your endorser does not have Singpass, you can download and print out a copy of the medical screening questionnaire for your endorser to sign by selecting "Download medical screening questionnaire"
