You are unable to change the endorser's email address online after you have submitted the questionnaire. You will then need to prepare a hardcopy questionnaire for your endorser. Please download and print the questionnaire from the CMPB website and have your endorser endorse on the hardcopy questionnaire. Please bring this hard copy questionnaire on your pre-enlistment medical screening appointment and submit it during your registration.
