Updated by MINDEF If you have opted for the "My endorser will complete the endorsement on a printout of Medical Screening Questionnaire" option, you may print the questionnaire at the last step on the confirmation page. Otherwise, your endorser may print the medical questionnaire from the endorser home screen. Please note that if you have submitted the questionnaire online, you cannot make amendments to the medical screening questionnaire after the submission. However, if you have any new medical conditions to declare following the submission of your medical screening questionnaire, you may do so during your consultation with the Medical Officer on the day of your pre-enlistment medical screening, and provide the relevant documents to support your declaration.
