What do I do if I have misplaced the information from the doctor to download the death certificate?

If it is within 30 days from the date of death, you can request the certifying doctor or hospital for the information.

If it is more than 30 days from the date of death, you may email ICA at ICA_DPU_RBD@ica.gov.sg for assistance.
