During your first Health Plan consultation, your doctor will create your Health Plan after getting to know you and your health and explaining what you can do to stay healthy or manage your chronic condition(s).

An annual Health Plan check-in is a follow-up review that occurs within the next calendar year after your first Health Plan consultation. This could also be carried out via a phone call, or when you next visit your doctor for other reasons. During the check-in, you and your doctor will discuss and update your existing Health Plan.
