Can I claim for subsidies for different chronic condition(s) under the Healthier SG Chronic Tier and existing CHAS Chronic Tier respectively during the same visit?
(E.g. Chronic condition 1 may be more beneficial under the Healthier SG Chronic Tier because it requires selected common chronic medications*, whereas chronic condition 2 may be better off under the existing CHAS Chronic Tier)
At each visit, you can choose to either tap on subsidies under the existing CHAS Chronic Tier or Healthier SG Chronic Tier, regardless of the number of chronic conditions seen in each visit. The remaining dollar subsidy limits will be pro-rated accordingly for the next visit if a different subsidy tier is chosen.
You may wish to consult with your Healthier SG clinic/doctor about switching between the existing CHAS Chronic Tier and the Healthier SG Chronic Tier, to cater for possible changes to your medication needs.
Healthier SG benefits and subsidies are available/accessible at your enrolled Healthier SG clinic.
*Additional subsides for Medication Assistance Fund (MAF) medications on the Healthier SG Medication List are available only for CHAS Blue and CHAS Orange cardholders (including PG/MG cardholders who also hold a CHAS Blue/Orange card).
