How do I assign a service provider for a direct referral?

• Navigate to the Services tab in the referral form

• Select Assign provider for the relevant service

• Select the service provider you wish to assign to

• Make sure the rest of the referral form is complete

• Submit the referral form

• The referral will be immediately assigned to the service provider upon referral submission

How do I change a service provider for a direct referral?

• Navigate to the Manage admission tab

• Note: You can only change a service provider if they have not yet acted on the referral yet (accepted/scheduled/admitted)

• Select another service provider

What if service provider is not available?

If they are available or do not have vacancy, you will receive an email informing you that the referral was not assigned. You will need to assign it out to another provider.

What are direct services?

Direct services include Hospice, Renal, ICS/ICS+, Psychiatric Rehab Home, Psychiatric Day Centre, Community Hospital, Transitional Care Facility.
