Can I not use the Sample Letter / Template to submit my complaint?

The Sample Letter / Template [DOCX, 60 KB] is a guide to draft your complaint as it contains all the relevant fields that an Inquiry Committee and a Complaints Committee would require to investigate your complaint. You are therefore strongly encouraged to adhere to this guide.


Please also note that handwritten complaint letters are not accepted and all complaints must be type-written in English.
