I noticed incorrect information on the death or stillbirth certificate. What should I do?
If it is within 6 months from the date of death or stillbirth, please notify the doctor or hospital to correct the information and issue a replacement certificate. Once a replacement certificate is issued download the replacement death or stillbirth certificate online, within 30 days from the date of issuance.
Please delete the version with incorrect information from your personal devices thereafter, as it will not be valid.
If it has been more than 6 months, please contact the Registry of Births and Deaths, ICA.
