How do I obtain an existing medical report certifying that the Donor has lost mental capacity?

You may submit a request for the Donor’s medical report to the hospital’s business office. Alternatively, you may call the general enquiry line of the hospital/hospice or write to the generic enquiry email address to ask for further details of the process.

The hospital/hospice may require supporting documents from you for verification before they will extend the report to you.
