For new Singapore Residents  who are automatically covered under MediShield Life, we implement a range of checks, including reviewing medical records after they are covered. They are also required to declare their health status upon receipt of the In-Principle Approval (IPA) letter from the Immigrated & Checkpoint Authority of Singapore (ICA). A copy of the form can be found by using this link (https://www.cpf.gov.sg/member/tools-and-services/forms-e-applications/health-declaration-pr-applicants)

If they are identified to have serious pre-existing conditions, they will be subject to the 30% Additional Premiums for 10 years. Those who make false health declarations will be guilty of an offence under the MediShield Life Scheme Act and will be liable to pay penalties.
