You were issued with more than 1 Service Injury (SI) Card when the diagnosis of your injury is too long to be contained within 1 card. As such, more than 1 SI card was issued. You are advised to bring all your SI cards for your medical appointment.    If you were issued with more than 1 SI card due to different injury claims, please bring along the correct SI card when seeking treatment for your injury.
