What can I do if the person who lost their IC is unable to report the loss online or in person?

The IC holder should report the loss of IC himself/herself as IC is a personal document.

However, you may report the loss of IC on behalf of the holder via FormSG if the holder is unable to report the loss of IC himself/herself due to medical reasons.

In your submission, you are required to attach the following documents:

  • A medical memo from a doctor outlining the individual's medical condition

  • A completed copy of the “Application for replacement of NRIC (loss)” form

  • A court order or Lasting Power of Attorney (LPA) appointing you as a donee (if the individual is assessed to be mentally incapacitated to make decisions), or a court-appointed deputyship
