How do I apply for a Certificate of Identity?

For application of Certificate of Identity, you are required to complete and submit the Certificate of Identity Form at ICA Services Centre in person.

You may walk in to the ICA Services Centre during our operating hours.

Please approach the Information Counter at Level 1 to obtain a queue ticket from our service ambassador.
