What should I do if any of my family members are unable to sign the consent form? Can I sign on his/her behalf?

It is important for an individual's personal data to be used only with one's own knowledge and consent. If he/she is at least 21 years old and has the mental capacity to understand the Terms of Consent, he/she should provide consent himself/herself.


If you have a family member in your application who is unable to provide consent (e.g. overseas, no authorised representative) to allow government schemes to access their personal data, please inform the government scheme to discuss how to proceed with your application.
