Where can I get further advice on how to work towards my health goals in my Health Plan after my first Health Plan consultation or annual Health Plan check-in?
Your Health Plan can be found on HealthHub and includes resources to support you in working toward your health goals, such as reminders for recommended screenings and vaccinations. From 27 July 2026, eligible enrollees will also be able to use the Health Plan Artificial Intelligence (AI) beta feature on HealthHub to generate exercise plans which are tailored based on their Health Plan, demographics, and personal preferences. If you would like to receive further support in achieving your health goals, you may speak with staff from a nearby Community Health Post (CHP). CHPs provide services such as lifestyle counselling and referral to community programmes, medication management, geriatric assessments and chronic care plan compliance monitoring. You are strongly encouraged to book a CHP appointment using the appointment booking forms linked below.
