What should I expect at my initial Health Plan consultation and annual Health Plan check-in?
During the Health Plan consultation at your enrolled Healthier SG clinic, the doctor will discuss your health and work with you to create a customised Health Plan, including:
Reviewing what Healthier SG screening tests and vaccinations* you need.
Discussing ways to improve your health, such as making lifestyle adjustments and how to manage your chronic conditions
Referring you to healthy lifestyle programmes conducted by community partners, and
Booking your next Health Plan consultation/ check-in
When discussing your Health Plan with your doctor, you may be advised to undergo additional medical tests, e.g. blood tests for chronic conditions, to monitor pre-existing conditions. Such tests are in addition to the specially subsidised Healthier SG screening tests* and will be charged at the clinic's prevailing rates.
*Eligible Singapore Citizens will enjoy subsidised screening tests and vaccinations at polyclinics and CHAS GP clinics, while eligible Permanent Residents enjoy subsidised screening tests and vaccinations at polyclinics. In addition, eligible Singapore Citizens who are enrolled with Healthier SG will enjoy special subsidies at their enrolled Healthier SG clinic.
Screening test eligibility is determined based on an individual’s age, sex, pre-existing conditions and last health screening date.
Vaccination eligibility is determined based on an individual’s age, sex, pre-existing medical conditions and vaccination history.
