I’ve submitted my Onboarding Questionnaire (OQ) on HealthHub, but I made a mistake, how do I change it? I entered a detail wrongly on my Onboarding Questionnaire and I can’t change it?
The OQ is designed for a one-time response. You may view the completed OQ through HealthHub, but you will not be able to edit the responses after submission.
If you have responded wrongly to the OQ, please inform your doctor during your first Health Plan consultation since the responses are meant to facilitate the consultation.
