If I change my enrolled clinic, can my new family clinic view my previous Health Plan?
Yes, your new family clinic will be able to access the Health Plan you co-developed with your previous clinic. This ensures continuity of care.
You can also view your Health Plan any time via the HealthHub app.
Remember, if you are generally healthy with no chronic conditions, it is recommended to check-in with your Healthier SG doctor once a year. If you have one or more chronic conditions, your doctor may recommend two or more check-ins within the year to better review and manage your condition(s).
