If you are aged 15 and above, the preschool will trigger the form to you as the main applicant. You may still access the LifeSG form via Singpass. The LifeSG form will be adjusted to collect consent from your parent/legal guardian.

If you are aged below 15, please inform the preschool. The form will still be triggered to you as the main applicant, but you will need to access the LifeSG form using your birth certificate number as you are not eligible for Singpass. The LifeSG form will also be adjusted to collect consent from your parent/legal guardian.
