To begin, your child must first be successfully enrolled in a preschool. The preschool will generate the LifeSG Form 1 for you in our system. If you are unable to locate the electronic record on LifeSG, ensure that your child has started attending classes at the preschool, as your preschool will only be able to trigger the LifeSG Form 1 on or after your child's date of enrolment. Additionally, contact the preschool to ensure that they have correctly recorded your NRIC, FIN or Passport number when generating the LifeSG Form 1.
