Where can I find information on the name of my Integrated Shield Plan and the remaining premium term of my Supplement Plan?

Please refer to the table of insurers and the steps to check the name of your Integrated Shield Plan as well as the remaining premium term of your Supplement Plan.

AIAPlease refer to your premium notice letter or the AIA+ app under 'Portfolio'
Great Eastern

For Integrated Shield Plan: View details in Great Eastern app under 'Policies' tab

For CareShield Life Supplement plan: Access via Great Eastern app > More > Document > Policy Contracts

HSBC Life

For hardcopy letters: Check Premium Breakdown Letter (renewal letter) sent 30 days before policy renewal

For e-letters via HSBC Life SG App:
1. Select Healthcare policy
2. Tap on Policy Documents
3. Tap on Select your policy
4. Select your policy from the list
5. Select documents to download
6. Save as PDF

Income

Access via:
1. Annual renewal letter and renewal certificate
2. me@income portal (https://me.income.com.sg/):
- Log in via Singpass or email + password + OTP
- Click 'View details' under Health Insurance tile
- Find relevant policy and click 'View details'

For Supplement Plan premium term: Check policy schedule from policy inception

PrudentialAccess via:
- Policy certificate
- Annual premium notice letter
- PRUaccess: Login > Policy Information > Click on respective policy number
Raffles Health Access via Raffles Connect app: 
- Go to 'Account' tab 
- Click on 'Insurance' to view your policy contract
SinglifeAccess via Singlife website:
- Login > MySinglife > MyDocuments 
 
If you cannot locate this information through these channels, please contact your insurer or financial advisor for assistance. They maintain your complete policy records and can provide these details.
