When do I need to submit the documents (Medical Assessment Report and employment information) to support my application for Dependants' Protection Scheme claim?

If you had applied for Dependants’ Protection Scheme claim through the CPF website, the medical institution that you have approached and your employer are required to submit the completed Medical Assessment Report and your employment information respectively to CPF Board within 90 days of your application. The Medical Assessment Report and employment information will be used to process claims under Dependants' Protection Scheme (DPS) and Home Protection Scheme (HPS), if you have applied for both. We will not accept Medical Assessment Reports submitted directly by CPF members.
Please note that your application will be cancelled if we do not receive the supporting documents by the given deadline.
To apply for withdrawal due to Reduced Life Expectancy (RLE) only, please write to us for assistance. If you are applying for DPS benefits only, you can submit the supporting document to Great Eastern Life directly.
