How can I claim Dependants' Protection Scheme benefits under medical grounds on behalf of my next-of-kin who lacks mental capacity?

As an appointed donee/deputy for your next-of-kin, you can claim his insurance benefits and withdraw CPF savings due to Reduced Life Expectancy by completing this form and submitting the required documents as listed in the form. The form will allow you to concurrently apply for both Dependants' Protection Scheme (DPS) and Home Protection Scheme (HPS) benefits, as well as withdrawal of CPF savings.
 
Upon receipt of the completed form and supporting document(s), CPF Board and Great Eastern Life (for DPS) will review the application and follow up with you.
