If you already have a Medical Assessment Report on hand, you can submit it to CPF Board through the link which will be provided after you submit an online claim form. The application form will allow you to concurrently apply for both Dependants' Protection Scheme (DPS) and Home Protection Scheme (HPS) benefits, as well as withdrawal of CPF savings. We will assess whether the report you have provided is sufficient and inform you accordingly.

If you are claiming for DPS benefits only, you may submit the report directly to Great Eastern Life (GEL). GEL will assess whether the report you have provided is sufficient and inform you accordingly.
