Change request can be raised through this form - https://go.gov.sg/brightchangerequest

You will be able to raise a change request for the following:

1. Change of Admission Date

2. Change of Discharge Date

3. Change of Subsidized License Beds/Approved Capacity

4. Cancel Admission

5. Change in Location Name

6. Change in Referral biodata (only applicable for IRMS)

7. Re-push Referral to ILTC Portal

Please alert your Reporting Officer to give his/her approval after you have submitted a change request. This is to ensure that the change request will reach AIC who will give the final approval before we can act upon the change request.
