Family may approach their loved ones' doctor at the hospital/polyclinic/private clinic where they are currently receiving treatment at, to assist with the application.

Alternatively, family may also approach the nearest AIC Link to discuss on the suitability of the service and in making an application for subsidised referrals. Some of the documents you will need to bring along includes:

· NRIC of the care recipient (front and back)

· Recent medical report or discharge summary
