To apply for Day Rehabilitation service, you may approach your loved ones' doctor at the hospital/ polyclinic/ private clinic where they are currently receiving treatment at, to assist with the application.

The cost of the Day Rehabilitation service will vary depending on your subsidy level. This is assessed based on the Per Capita Monthly Household Income Requirement for subsidies for Non-Residential Services under Household Means-Testing. If you have difficulty paying for the service, please speak to your service provider or the Medical Social Worker in the hospital or polyclinic.
