How do I apply for community rehabilitation service and what is the cost?

How to apply

Talk to your doctor and/ or therapist at the hospital or polyclinic. They can help submit the application online. You may also visit an AIC Link to get help with the application.

Cost

The cost, before any government subsidies, is:

  • Active rehabilitation: Starts from $55 per session

  • Maintenance rehabilitation: Starts from $56 per session

Transport to and from the centre and home is available at an additional cost.

To qualify for government subsidies, your loved one will need to be a Singapore Citizen or PR. Subsidies are also means-tested (ie there is an income criteria).
