You may carry out the following steps:

Please log in and click on “Enquire CPE Claims” to find the application.
Then click on the word “here” under “Remarks” to attach the supporting document.

Please include your documents in “pdf” or “jpg” format and ensure that it does not exceed more than 1MB.
