Please indicate the clinic that you are most frequently practising at, when you submit an application to Update Particulars. You may provide the following information for your locum practice.


  • Working full-time or part-time (if part-time, please indicate no. of hours per week)

  • Name/Address of Institution/Organisation (this refers to the clinic that you are most frequently working in) 

  • Designation/Appointment

  • Department/Division

  • Effective Date
