I misplaced my blood test forms, what should I do?

Please inform your TCs at least one day before coming down for your blood test. Patients who arrive without their blood test forms may also inform the transplant centre clinic assistants/ nurses to seek assistance with reprinting their forms. However, do note that there will be additional waiting time incurred to check on the necessary blood tests required and reprinting of the forms.
