Please bring along the original request form and any previous X-rays, CT, MRI or Ultrasound images performed in any private clinic/outside institutions that you may have. Please arrive 20 minutes before your appointment time for registration. If you are late, your appointment may be rescheduled to another day/time.

If you are covered by your company or any other payment schemes:

Please provide the Letter of Guarantee (LOG) and/or any other supporting documents during registration.

For referral by SAF

Please provide the Letter of Identity (LOI) issued by CMPB, dated same day as procedure date or SAF Identity Card (11B), whichever applicable. Submit to our staff on the day of procedure.
