How do I submit an assessment report?
The assessment reports will be routed for supervisor’s completion every 6 months for C-reg dentist and every 12 months for Part II OHT, or whenever required.
Supervisors may refer the Guidebook on supervision of dental practitioners on how to complete and submit the assessment report. The Guidebook can be downloaded on the Downloads page under Registration-related Forms.
