How do I lodge a complaint against a registered optometrist or optician?
Your complaint must be type-written in English and supported by a Statutory Declaration (SD) which must be affirmed before a Commissioner for Oaths / Justice of Peace. A SD is a written statement of facts that is signed and affirmed by the person making the declaration that the contents in the statement are true and accurate to the best of his knowledge and belief.
Once the SD has been duly affirmed / sworn, you can submit the SD and your complaint letter via email:
| What to submit | Where to submit to: |
